The document was written with a focus on conventionally reinforced slabs and not post-tensioned slabs. If there are no tendons in the slab, the balance loading effects would be 0 and the use of All Dead LC or Dead + Balance LC would not matter.
If the slab was post-tensioned, however, the exclusion of the balance loading effects would have a significant effect on the load history deflections. I would expect either extremely high deflections or a convergence problem. The best modeling practice is to include the Balance Loading in all combinations referenced in the load history steps. This would prevent that problem from occurring.
